 This is an informational patch for the Decision Support Tool application
 version 1.0.04 Build 14. There is no VistA component to this new build. 
 This patch is for informational purposes only and defines the scope of the  
 changes that have been made including defects that have been resolved in DST. 
 ******************************************************************************
 
 Decision Support Tool version 1.0.04 has the following functionality changes
 and defect fixes:
 
 1. #1010791: Display specific Facility types associated with Specialty Care
 Consults:
    - Decision Support Tool will display only the following Facility type for
      Specialty Care consults to determine Drive Time more accurately -
      - VA Medical Center (VAMC),
      - Health Care Center (HCC),
      - Medical Specialty Community-Based Outpatient Clinic (MS CBOC).
    
 2. #1010786: Display accurate Drive Time information associated with the
 facility for the selected Clinical Service:
    - Decision Support Tool will perform a look up within 100 or 50 miles radius
      to display Drive Time for Specialty Care.
    - Decision Support Tool will perform a look up within 40 or 20 miles radius
      to display Drive Time for Primary Care.  
 
 3. #1009783: Restoring Consult Name associated with Signed Consult:
    - User will not see the new consult name in Consult section on Decision
      Support Tool dashboard for signed consult that has been forwarded.
