If a Medication Routes file abbreviation for a Clinic Med or Inpatient Med order matches a Medication Instruction file Name, the Essential Med List for Review Report (EMLR) will expand the Med Route using the Medication Instruction File expansion. This may result in incorrect or unintended clinical terminology being displayed for the order. INC9511598 Med Instructions Display incorrectly with Clinic Medications Problem: ------- For Clinic and Inpatient Medications, the Essential Med List for Review Report (EMLR) in CPRS applies the MEDICATION INSTRUCTION file (#51) expansion to all order text. If the MEDICATION ROUTES file (#51.2) contains an entry in the Abbreviation field (#1) identical to an entry in the MEDICATION INSTRUCTION file Name field (#.01), the MEDICATION INSTRUCTION file Expansion field (#1) will be applied to the Med Route field (#3) of the Unit Dose multiple (#62) of the PHARMACY PATIENT file (#55). This may result in incorrect or unintended clinical terminology being displayed for the order. Resolution: ---------- This patch adds a new report which displays matches between certain fields in the MEDICATION INSTRUCTION file (#51) and the MEDICATION ROUTES file (#51.2). The Name field (#.01) in the MEDICATION INSTRUCTION file (#51) and Abbreviation field (#1) of the MEDICATION ROUTES file (File #51.2) may have entries that use similar terminology and abbreviations but with different clinical meanings. The MED INSTRUCTION MED TERM ROUTE REPORT option identifies instances where a MEDICATION INSTRUCTION file Name and a MEDICATION ROUTES file Abbreviation are identical to facilitate review and correction of clinically inconsistent terminology between the two files. Local Pharmacy Informaticists should review and evaluate this report for possible corrective action to ensure that reports which use the expansion from the MEDICATION INSTRUCTION file are clinically valid when the data originates in the MEDICATION ROUTES file. This patch adds the Med Instruction Med Term Route Report to the Medication Instruction Management menu (PSS MED INSTRUCTION MANAGEMENT) and the Medication Routes Management menu (PSS MEDICATION ROUTES MGMT). Upon running the report, sites should refer to the Pharmacy Data Management User Manual located on the VistA Software Document Library for the dedicated Pharmacy website to discuss the potential options for file management or to see previous site's decisions on how similar entries were handled. For reference, the dedicated Pharmacy website is located at this URL: https://dvagov.sharepoint.com/sites/VHAPBM/DiscussionBoards/Lists/Prescrip tion_Dose_Dispense_Unit_Discrepancy_Report/AllItems.aspx Documentation updates are applied to the Pharmacy Data Management User Manual as well as the Pharmacy Data Management Technical Manual/Security Guide.
