The other patches (TIU*1.0*290, WV*1.0*24, GMTS*2.7*67, DG*5.3*932, PXRM*2.0*45, GMRA*4.0*53, GMRC*3.0*88, PSO*7.0*477, and OR*3.0*377) are being released in the KIDS multi-package build CPRS V31B REQUIRED PATCHES 1.0. This patch installs the following document class and note titles: System for Mammogram Tracking (SMART) Notifications SMART is a new optional alert processing system for breast imaging exam follow-up. It assists providers in documenting the results of breast imaging exams, communicating those results to the patient, and scheduling follow-up and/or future exams. The note titles below are used to capture the follow-up documentation. TIU DOCUMENT DEFINITION (8925.1) The new titles are attached to the new document class. NAME TYPE ---- ---- SMART NOTES DOCUMENT CLASS SMART BREAST IMAGING FOLLOW-UP TITLE SMART PATIENT NOTIFICATION TITLE SMART OUTSIDE BREAST IMAGE RESULTS TITLE Women's Health High Risk Medications The high risk medications functionality enables providers to document a patient's pregnancy and lactation status in the Women's Health software package. One feature of that functionality tracks the pregnancy and lactation status in other VistA software packages (such as Laboratory and Patient Care Encounter). When there is a mismatch, an alert is generated notifying case managers of such and will assist them in updating the status in the Women's Health software package. The note titles below are used to capture this status change. TIU DOCUMENT DEFINITION (8925.1) The new titles are attached to the WOMEN'S HEALTH NOTES* document class. *The WOMEN'S HEALTH NOTES document class should already be present in File #8925.1. If it is not present, the installation will abort so that it may be added. NAME TYPE ---- ---- PREGNANCY STATUS UPDATE REVIEW TITLE LACTATION STATUS UPDATE REVIEW TITLE Lifetime Health Monitoring TIU DOCUMENT DEFINITION (8925.1) The new title is attached to the SECURE MESSAGING DOCUMENTS document class. NAME TYPE ---- ---- HEALTHELIVING ASSESSMENT SUMMARY TITLE This patch introduces new functionality that will track the copying and pasting of text within documents in the Computerized Patient Record System (CPRS). This patch also introduces new functionality that will allow the TIU software package to notify other software packages when actions are taken on documents. Before taking an action on a document, it will poll other software packages for informational messages that those packages need to display to the user taking the action on the document. Lastly, this patch introduces new functionality to prevent edits to templates and template fields that are used in consult requests. The TIU TEMPLATE CONSULT LOCK option has been added to the TIU IRM TEMPLATE MGMT menu. This menu allows the user to set System-level values for the TIU TEMPLATE CONSULT LOCK parameter. Adding a TIU consult template to that parameter will prevent any edit to that template and all its associated template fields when using the GUI Template Editor. This locking is only applicable to templates that are linked to a consult service. Defect Tracking System Ticket(s) & Overview: -------------------------------------------- 1. Remedy INC000000069650 - Resident Documentation not requiring Co-Sig Remedy Duplicate INC000000069993 ALERTS FOR COSIGNER NOT WORKING W UPLOAD Problem: -------- If the author of a TIU Progress Note requires co-signature and the note is uploaded into TIU without an Expected Cosigner, no Missing Field alert is generated. Resolution: ----------- A Missing Field alert is now generated. 2. Remedy INC000000298417 2 Chief,MIS is unable to amend a discharge summary Problem: -------- If an authorized person attempts to amend an un-cosigned document, they are correctly refused permission to amend but they receive the message: "Only SIGNED Documents may be amended." This message implies that the user should be able to amend the document. In fact, TIU (Text Integration Utility) permits only COMPLETED documents to be amended; un-cosigned documents may NOT be amended. Resolution: ----------- The message now reads: "Only COMPLETED Documents may be amended." 3. Remedy INC000000089942 5 DISCHARGE SUMMARY "REF DATE" Remedy INC000000290691 (Duplicate) DISCHARGE SUMMARY DATE IN FUTURE Problem: -------- Users are not permitted by policy to upload Discharge Summaries with Dictation Dates in the future into TIU. Currently, the TIU Upload option goes ahead and uploads such Discharge Summaries. Resolution: ----------- The upload option now aborts the upload if the Dictation Date of a Discharge Summary is in the future. 4. Remedy INC000000828109 - Undefined for Proxy User Problem: -------- Some systems from outside of VA (in this case the CERNER Lab System) utilize proxy users in the VA NEW PERSON FILE [#200]. Proxy users do not have Electronic Signatures. In fact for proxies, the whole 20th node of file 200 may not exist. TIU does not check whether node 20 exists. This generates an undefined error when TIU attempts to display a Proxy document. Resolution: ----------- The OUTPUT TRANSFORM for the EXPECTED SIGNER field (#1204) in TIU DOCUMENT FILE [#8925] now checks for the existence of the 20 node in file 200 before going on to check for the existence of an Electronic Signature in file 200. If either check fails, TIU uses the name itself of the proxy in file 200 rather than its Electronic Signature name. Test Sites: ---------- Atlanta VA Health Care System (Decatur, GA) Hampton VA Medical Center (Hampton, VA) Northport VA Medical Center (Northport, NY) Louis Stokes Cleveland VA Medical Center (Cleveland, OH) Documentation Retrieval Instructions: ------------------------------------- Sites may retrieve the software and/or documentation directly using the Secure File Transfer Protocol (SFTP) address as directed by the CPRS Implementation team. Installation instructions will be distributed when your site begins the implementation process. Documentation can also be found on the VA Software Documentation Library at: http://www.domain.ext/vdl/ The updated documentation will not be on the vdl until after the phased deployment has completed. Title File Name FTP Mode ----------------------------------------------------------------------- Clinical Coordinator and tiuum.pdf/doc Binary User Manual TIU Technical Manual tiutm.pdf/doc Binary Patch Installation: ------------------- Pre/Post Installation Overview: ------------------------------- This patch contains an environment check routine (named TIUE290) that will check your system for any name collisions with the incoming document class and note titles previously mentioned. The pre-installation routine will use the Reminder Exchange installation utility to remove any previous versions of the Reminder Exchange file used to install the note titles. The post-installation routine will use the Reminder Exchange installation utility to install the document class and note title entries into the TIU DOCUMENT DEFINITION file (#8925.1) under the PROGRESS NOTES document class. It will set the CLASS OWNER for the new document class and note title entries to CLINICAL COORDINATOR. Then it will attempt to map the new SMART and Women's Health High Risk Medications note titles to the TIU VHA Enterprise Standard title of WOMENS HEALTH NOTE and map the new Lifetime Health Monitoring project note title to the TIU VHA Enterprise Standard title of RISK ASSESSMENT SCREENING NOTE. Finally, it will attempt to activate the document class and note titles. If the mapping or activation fails for any given item, sites can manually complete those activities using the TIU Document Definitions (Manager) option [TIUF DOCUMENT DEFINITION MGR]. The post-installation routine will also attempt to set values into the new TIU TEMPLATE CONSULT LOCK parameter at the System level. The process will look for any TIU TEMPLATE (#8927) entries where the NAME (#01.) begins with the text "NON VA CARE HCPS ". Only templates that are found to match that text AND are also linked to consult services via the LINK field (#.19) will be added to the parameter. Installation Instructions: -------------------------- Please refer to the "CPRS v31b Deployment, Installation, Back Out and Rollback Guide" for installation and set-up information. This document is exported as OR_30_377_IG.doc and OR_30_377_IG.pdf and is included in OR_30_377.ZIP and will be emailed to the installation point of contacts by the CPRS Implementation team. Post-Installation Instructions: ------------------------------- After a successful installation and all components are properly mapped and activated, the installer may safely delete routines TIUE290 and TIUP290.
