Patient Safety Issues (PSIs): ============================ HITPS-7744 Defect Tracking System Ticket(s) & Overview: ============================================ INC16713088: Pending Inpatient Medications and Auto D/C Rules Problem: ------- At a site, a patient did not get a critical inpatient medication reinstated since it was still PENDING and had not been processed by Pharmacy before the discharge from Observation movement was done. There was no warning to the ordering provider of the discontinuation of that critical inpatient medication or any warning to the Inpatient Pharmacy staff. The CPRS Technical Manual: GUI Version and CPRS Technical Manual: List Manager Version have been updated to clarify the CPRS functionality for reinstating orders if the Auto D/C rule for Discharge is set to "Except from observation: IF READMITTING". The rule only works for inpatient medication orders that are in the "ACTIVE" status and not Inpatient medications orders in "PENDING" status. Inpatient medications orders in "PENDING" status are discontinued when a patient is discharged from OBS and then admitted to a regular ward. Resolution: ------------ The following verbiage has been added to the CPRS Technical Manual: GUI Version and CPRS Technical Manual: List Manager Version. NOTE: Medication orders in a "Pending" status cannot be exceptions to Auto-Discontinue rules for a Discharge from an Observation Treating Specialty. If this field is set to "YES", or set to "If Readmitting" and the user inputs "YES", a pending medication order will still be Auto-Discontinued upon Discharge from an Observation Treating Specialty. Only medication orders in an "Active" status will be reinstated if the Except from Observation field is set to "YES" or if the user enters "YES" to the question, "Will the patient be re-admitted immediately?" This field can be set in the following ways: > Yes - if you set the field to yes, a discharge from an observation treating specialty will always be an exception to this rule (should not cause an order to be discontinued). > No - if you set this field to no, this rule will be applied regardless of whether the patient is discharged from an observation treating specialty. > If Readmitting - If you set this field to If Readmitting, the user will be prompted to enter whether the patient will be immediately readmitted. If the user answers yes, the order will not be automatically discontinued. If the user answers no, the rule will be applied.
