Microbiology reports might not display the accurate status of a test. 2. Unverified Microbiology results are visible in Computerized Patient Record System (CPRS) reports. 3. VistA reports might not list an accurate status for Microbiology accessions. 4. If a session times out while entering Microbiology sensitivity results, the user may not be aware that the sensitivity results were not stored. EFFECT ON BLOOD BANK FUNCTIONAL REQUIREMENTS: Patch LR*5.2*536 does not alter or modify any software design safeguards or safety critical elements functions. RISK ANALYSIS: Changes made by patch LR*5.2*536 have no adverse effect on Blood Bank software functionality, therefore RISK is none. VALIDATION REQUIREMENTS BY OPTION: Because of the nature of the changes made, no specific validation requirements exist as a result of installation of this patch. Patient Safety Issues (PSIs): HITPS-6762 (INC10855390) HITPS-6945 (INC10856615) HITPS-7194 (INC10369256) Defect Tracking System Ticket(s) & Overview: 1. INC10855390: Microbiology Test Status Display Issue Problem: -------- If a Microbiology accession contains more than one test, and one test is canceled, both the VistA and the CPRS Microbiology reporting will not display an accurate status for all tests on the accession. Either all tests will display as "completed" or all tests will display as "canceled". Resolution: ----------- Modify routines LR7OSMZ1 and LRMIPSZ1 to display an accurate status for each test on a Microbiology accession. Routines: LR7OSMZ1, LRMIPSZ1 2. INC10856615: Microbiology Unverified Results Display on Reports Problem: -------- If a previously verified Microbiology accession is in the process of being edited, results being entered are visible in CPRS during the editing session before the results have been verified. Resolution: ----------- Modify Microbiology results entry logic to delete the previous date/time stamp from the LAB DATA (#63) file which denotes the results were verified. This alerts CPRS reporting logic that the Microbiology results have not been verified and should not display. The date/time stamp will be populated with an updated date/time when the user re- verifies the results. NOTE: Since the date/time stamp will be deleted when the Results entry [LRMIEDZ] option is selected, results will no longer be available without entering a new "[area] RPT DATE APPROVED". Users who view results via the Results entry [LRMIEDZ] option should cease this practice. Other options can be used to view results, such as: > All results for selected accessions [LRMIPLOG] > General report for selected tests [LRGEN] > Interim report for an accession [LRRP1] > Lab tab in CPRS Routine: LRMIEDZ2 3. INC10369256: VistA Report Issue for Pending Microbiology Accessions Problem: -------- The "Incomplete test status report", "Long form accession list", and "Short accession list" either do not list or list an incorrect status for Microbiology accessions that have had the "[test name] completed:" prompt answered but not the "[area] RPT APPROVED DATE" prompt. Results will not display in CPRS if the "[area] RPT APPROVED DATE" prompt has not been answered, which means the accession is incomplete. Resolution: ----------- Modify routines LRWRKINC, LRWRKLST, and LRLSTWRK to correctly display any Microbiology accessions that do not have the "[test name] completed:" prompt and/or the "[area] RPT APPROVED DATE" prompt answered. Routine: LRWRKINC, LRWRKLST, LRLSTWRK 4. INC9599852: Microbiology Sensitivity Timeout Issue Problem: -------- If a user allows a session to time out while entering Microbiology sensitivity results manually, the sensitivity results will not be saved in the patient's record. The user might not be aware that the results have not been saved. Resolution: ----------- If a session times out, display a message alerting the user that: (1) some results may not have been saved into the patient's record; (2) results must be double-checked and possibly re-entered. The user is then taken back to the "Select ORGANISM:" prompt. Routine: LRMIBUG Test Sites: ----------- James J. Peters VAMC (Bronx, NY) VA San Diego HCS (San Diego, CA) VA Pittsburgh HCS (Pittsburgh, PA) Software and Documentation Retrieval Instructions: -------------------------------------------------- The software for this patch is being released in a PackMan message. Patch Installation: ------------------- Pre-Installation Instructions: This patch may be installed with users on the system although it is recommended that it be installed during non-peak hours to minimize potential disruption to users. This patch should take less than 5 minutes to install. NOTE: Routines LR7OSMZ1, LRMIPSZ1, and LRMIEDZ2 had previously been released in LR*5.2*533. That patch status was later changed to "Entered In Error", so sites should have backed the routines out. If the routines were not backed out, there is no need to back them out prior to the install of LR*5.2*536 since the routines will be overlaid. The before checksum values for the three routines will display unexpected "before install" values in the Verify Checksums in Transport Global option if LR*5.2*533 was not backed out. Installation Instructions: 1. Choose the PackMan message containing this build. Then select the INSTALL/CHECK MESSAGE PackMan option to load the build. 2. From the Kernel Installation and Distribution System Menu, select the Installation Menu. From this menu, A. Select the Verify Checksums in Transport Global option to confirm the integrity of the routines that are in the transport global. When prompted for the INSTALL NAME enter the patch or build name. (ex. or XXXXX BUILD X.X) NOTE: Using will not bring up a Multi-Package build even if it was loaded immediately before this step. It will only bring up the last patch in the build. B. Select the Backup a Transport Global option to create a backup message of any routines exported with this patch. It will not backup any other changes such as DDs or templates. C. You may also elect to use the following options: i. Print Transport Global - This option will allow you to view the components of the KIDS build. ii. Compare Transport Global to Current System - This option will allow you to view all changes that will be made when this patch is installed. It compares all of the components of this patch, such as routines, DDs, templates, etc. D. Select the Install Package(s) option and choose the patch to install. i. If prompted 'Want KIDS to Rebuild Menu Trees Upon Completion of Install? NO//', answer NO. ii. When prompted 'Want KIDS to INHIBIT LOGONs during the install? NO//', answer NO. iii. When prompted 'Want to DISABLE Scheduled Options, Menu Options, and Protocols? NO//', answer NO. Back-out/Roll Back Plan: ------------------------ In the event of a catastrophic failure, the Facility CIO may make the decision to back-out the patch. It is imperative that you have performed a backup of the routines included in this patch prior to installation. The back-out plan is to restore the routines from the backup created. No data was modified by this patch installation and, therefore, no rollback strategy is required. To verify the back-out completed successfully, ensure the checksums match the pre-patch checksums from this patch description.
