 2.  Patient Address Update option is not displaying correctly.

 1)  INC000000626738    Field #24-Appointment Type Sub-Category in the Patient
                        File (#2) is not assigned    
  
 Problem:
 --------
 The APPOINTMENT TYPE SUB-CATEGORY field (#24) of the APPOINTMENT sub-file 
 (#2.98) of the PATIENT file (#2), which is a pointer to the SHARING 
 AGREEMENT SUB-CATEGORY file (#35.2), is not assigned properly.
  
 When making an appointment using the Appointment Management [SDAM APPT 
 MGT] option, if the user enters SHARING AGREEMENT as the Appointment Type,
 the Registration Application Program Interface (API) will present an 
 additional prompt for the user to enter the Appointment Type sub-category.
 If there is no selection made, the application times out or user enters an
 (^) up-arrow to exit prompt, the API returns a value of zero, which is 
 filed into APPOINTMENT TYPE SUB-CATEGORY field (#24).
  
 Resolution:
 -----------
 Modified routine ^DGSAUTL to return a Null value when there is no selection
 made, application times out or user exits prompt with the "^" selection.
  
  
 2)  INC000000772240    Patient Address Update option 
   
 Problem:
 --------
 When using the Patient Address Update [DG ADDRESS UPDATE] option the city,
 state and zip code appears twice in the view for the Temporary Address.
 Also, after the county name there is a () showing.
  
 Resolution:
 -----------
 Modified routine ^DGADDUTL to only display () if there is a Catchment Code
 for that county and to only display the address once.
        
  
  
 Installation Instructions:
 --------------------------
 This patch may be run with users on the system, but it is recommended
 that it be queued to run after normal business hours.
   
 Installation will take less than 5 minutes.
 Suggested time to install: non-peak requirement hours.
   
      1. Use the INSTALL/CHECK MESSAGE option on the PackMan menu.
   
      2. From the Kernel Installation & Distribution System menu, select
         the Installation menu.
   
      3. From this menu, you may select to use the following options (when
         prompted for INSTALL NAME, enter DG*5.3*872).      
  
         a. Backup a Transport Global - this option will create a backup
            message of any routines exported with the patch. It will NOT
            backup any other changes such as DDs or templates.
         b. Compare Transport Global to Current System - this option will
            allow you to view all changes that will be made when the patch is
            installed.
         c. Verify Checksums in Transport Global - this option will ensure
            the integrity of the routines that are in the transport global.
         d. Print Transport Global - this option will allow you to view
            the components of the KIDS build.
      
      4. Use the Install Package(s) option and select the package DG*5.3*872
   
      5. When prompted "Want KIDS to INHIBIT LOGONS during install ? NO//"
         respond No.
   
      6. When prompted "Want to DISABLE Scheduled Options, Menu Options,
         and Protocols? YES//" respond Yes.
  
         When prompted, 'Enter options you wish to mark as 'Out Of 
         Order':', enter:
              
                   Appointment Management  [SDAM APPT MGT]
                   Patient Address Update  [DG ADDRESS UPDATE]
  
         When prompted, 'Enter protocols you wish to mark as 'Out Of
         Order':', press enter.
