The generated HL7 message will route to one of six VDIF Regional Routers. Six GMRC IFC Logical Links are included in this patch and are designated "GMRC IFC1" through "GMRC IFC6". IFC messages from converted sites to unconverted sites will be routed by VDIF to the appropriate VistA and that routing is not impacted by this patch. The Master Patient Index (MPI) is queried for information regarding a patient at the treating facility including whether the treating facility has been converted to Cerner and if the patient is known to Cerner. Patients at a converted site but unknown to Cerner will be added as will patients unknown to the treating facility using new proxy add functionality added as part of MPIF*1.0*73. If there is a need to change the automatically assigned VDIF Logical Link this information is held in a new Parameter, GMRC IFC REGIONAL ROUTER which contains the name of the Logical Link to be used when a converted site is detected by the IFC system. Note that the patch installs six (6) Regional VDIF Logical Links, GMRC IFC1 through GMRC IFC6, but each site will only use the one that is relevant for your site depending on the Region your site resides in. During development and testing, issues were discovered with prosthetic IFC orders and have been addressed in the patch. Issue/Defect: OTHER+13^GMRCIACT ^GMR(123,370322041,12) This patch addresses the following defect that occurs when Cerner sends an update via HL7 to a previously filed Vista Prosthetics IFC order: ERROR TEXT: OTHER+13^GMRCIACT ^GMR(123,370322041,12) The error occurs when the Vista Consult Request Tracking Interfacility Consult (IFC) processing logic assuming the incoming Cerner Order number is a local Consult order number. Prior to EHRM Prosthetics IFC, there were always two different Vista stations involved in an IFC transaction. With the implementation of the EHRM Prosthetics IFC interface, both the Placer System (Cerner) order number and the Filler System (Vista Prosthetics) order number are associated with the same Vista station. As a result, checking the station number associated with the consult order is not sufficient to determine which order (Placer or Filler) is the local order. To address this issue, Consults Request Tracking will use the Filler System order number as the local order number if the following are true: 1) The Filler System station is equal to the Placer System station -AND- 2) The incoming IFC message is a PROSTHETICS IFC order -AND- 3) The Filler System station is equal to the local system's station -AND- 4) The Filler System order exists in local REQUEST/COSULTATION file (#123). ---------------------------------------------------------------------- Issue - Prosthetics IFC DC Incoming Prosthetics IFC 'Discontinue' HL7 messages (ORC-1="OD" and ORC-5="DC") are posted in both Vista Consults Request Tracking (CRT) and Prosthetics Suspense. Vista Consults correctly changes the consult status to Discontinued, but the Prosthetics Suspense entry remains in an Open status, and creates a duplicate Prosthetics Suspense entry, changing only the "Type" to "DISCONTI" and the "Description" to "****DISCONTINUED****". The original suspense request is not changed other than the Discontinue action displaying in the record's activity log. With this patch, a change is made to the Prosthetics IFC processing. After Consults Request Tracking IFC HL7 processing is complete, a check will be done to determine if the orders are EHRM Prosthetics IFC orders, and if they are, a new routine is invoked from the inbound IFC processing routine to process the Prosthetics IFC order. If the incoming order is not an EHRM Prosthetics IFC order, processing continues as it did prior to the patch. After installation of this patch, a discontinue action received for a EHRM Prosthetics IFC suspense entry will result in the status of the Prosthetics suspense entry will change to 'CANCELLED'. INC12134027 - This ticket was opened by Spokane during testing of GMRC*154 v20 in prod. The consult does not display at the consulting site if an intra-facility consult is forwarded to an IFC service for a patient that is not registered at the consulting site. This situation was caused by the logging of an Unknown Patient error (201) in the IFC MESSAGE LOG file when an intrafacility consult was forwarded to IFC. The 201 error subsequently blocked the forwarding due to existing code which was designed to send the new consult and any intervening activities between the new consult and the forwarding activity. The result was that the 201 was never removed form the IFC MESSAGE LOG file so the forwarding of the consult was permanently blocked. The remedy was to remove the logging of the 201 error from its original place in the ROUTE API (whose purpose was to return the logical link of the receiving facility) and instead to log it only in the API that creates the original consult.
