The primary changes are updates to the Digital Imaging and Communications in Medicine (DICOM) Export, which is used to fetch DICOM objects from the local VistA Imaging storage and send them to a DICOM storage class provider (SCP). Currently the DICOM Exporter supports two different modes for selecting the studies. The first menu option, 2-8-1 Select DICOM Images for Transmission, allows you to select individual radiology, consult, or lab images/studies for transmission. The second option, 2-8-5 Batch Export VistA Radiology Images, allows groups of only radiology studies to be exported as a batch. All export requests are placed into the DICOM OBJECT EXPORT file (#2006.574) and the 2-8-2 option Transmit DICOM Images to a Storage SCP would handle the DICOM C-Store to the designated SCP. Enhancements to the DICOM Export are: 1. IMAG-2703 - Exporting new SOP (P34)class database DICOM objects 2. IMAG-2704 - Batch exporting of consult images 3. IMAG-2705 - Batch exporting of anatomic pathology images 4. IMAG-2727 - Export images using a list of accession numbers 5. IMAG-2728 - The ability to place a study in the export queue on hold and the ability to release the hold 6. IMAG-2729 - The ability to set the retransmit time for items in the export queue with a XMIT and FAIL status 7. IMAG-2720 - The ability to set the number of DICOM objects sent at a time during an export 8. IMAG-2731 - Improved batch export Statistics 9. IMAG-3043 - IGNORE and NOT ON FILE States 10. Defect IMAG-2736 (INC16125090) - DCF_STORE_SCU uses only original transfer syntax Problem: -------- Some images fail to send with the transfer syntax set in the DCF_STORE_SCU configuration. The "association_ts_uid" is set to the original transfer syntax of the DICOM object being exported in the Laurel Bridge DCF_STORE_SCU configuration file. This caused problems when the store SCP could not handle the original transfer syntax. Resolution: ----------- Remove the setting of the configuration variable "association_ts_uid." This allows DCF_STORE_SCU to negotiate the transfer syntax with the store SCP 2. Defect IMAG-2738 - Inconsistent variable names Problem: -------- Several variable names in the code were inconsistent between routines. Resolution: ----------- The following variable names were modified to be consistent throughout the code. ACCNUMB to ACNUMB BATCHSIZ to BATCHSIZE SORTORDR to SORTORDER PRIOR to PRIORITY STUID to STUDYUID S0 and S1 to D0 and D1 3. Defect IMAG-2739 - Improved performance when starting LDGW processes Problem: -------- The original design of the DICOM Gateway closed the TCP/IP connections before starting any process. This caused a five second delay each time a process was started to establish a new Remote Procdedure Call (RPC) session. Resolution: ----------- Updated routine MAGDMENU and MAGOSFIL to not closing the TCP/IP connection but to use the existing RPC session. 4. Defect IMAG-2740 - DICOM Q/R RPC Error when running a Q/R with "Use VistA Patient Lookup" selected Problem: -------- A DICOM Q/R user on a DICOM Gateway without an RPC connection gets an RPC error if "Use VistA Patient Lookup" is selected Resolution: ----------- The prompt is not presented if the user doesn't have an RPC connection. 5. Defect IMAG-2741 - DICOM GMRC TEMP LIST file (#2006.5839) "B" cross-reference not deleted Problem: -------- The "B" cross-reference of the DICOM GMRC TEMP LIST file (#2006.5839) has never been deleted. Resolution: ----------- The MAGDHOW0 routine was updated delete the "B" cross-reference when the entry is removed. 6. Defect IMAG-2742 - Images remaining in the DICOM GMRC TEMP LIST file indefinitely Problem: -------- Images are not attached to completed consults. When the images are acquired, they are attached to the DICOM GMRC TEMP LIST file (#2006.5839) and then attached to the next Text Integration Utilities (TIU) note when the diagnosis of the images was entered. The intent of this process was to provide the foundation for the following wound care workflow. 1. Consult for wound care is entered into Computerized Patient Record System (CPRS). 2. First set of images acquired and placed into DICOM GMRC TEMP LIST (#2006.5839). 3. Provider reviews the images, renders a diagnosis, enters it into CPRS, signs it, and completes the consult. 4. At intervals, wound images are acquired at the patient's residence and sent to the VA. a. Images are placed into the DICOM GMRC TEMP LIST. b. Provider reviews the images and uses CPRS Action: Consult Results, Complete/Update Results. to create a new TIU note, renders a diagnosis, enters it into CPRS, signs it, and completes the consult. 5. Step 4. is repeated as many times as necessary. The problem is caused by other types of consults being processed after the consult was completed, and they would remain in the DICOM GMRC TEMP LIST file. Resolution: ----------- Remove the check for if the consult is completed and attach the image to the last TIU note of a completed consult. This requires a change to the "wound care" workflow. 1. Consult for wound care is entered into CPRS. 2. First set of images is acquired and placed into DICOM GMRC TEMP LIST (#2006.5839). 3. Provider reviews the images, renders a diagnosis, enters it into CPRS, signs it, and completes the consult. 4. The provider must then use CPRS Action: Consult Results, Complete/Update Results. to create a new TIU note for the next set of images a. At intervals, wound images are acquired at the patient's residence and sent to the VA. b. Images are placed into the DICOM GMRC TEMP LIST. c. Provider reviews the images, renders a diagnosis, enters it into CPRS, signs it, and completes the consult. d. The image in the DICOM GMRC TEMP LIST is then attached to the latest TIU note. e. The provider then uses CPRS Action: Consult Results, Complete/Update Results. to create a new TIU note for the next set of images. 5. Step 4. is repeated as many times as necessary. 7. Defect IMAG-3042 - Study Tracker "CHK" option does not handle Lab Anatomic Pathology Problem: -------- The current option in VistA Study track Check a Radiology or Consult Study for Images only provides information for Radiology exams and Consults. Entering an Anatomic Pathology accession number does not return anything. Resolution: ----------- Code was added to return imaging information for a given laboratory exam. 8. Defect IMAG-3045 (INC20796501) - Unable to select Service Class Provider in VistA Problem: -------- Users at Orlando are unable to select a service class provider when running the Study Tracker Query and Retrieve options in VistA. The option is there when running the Study Tracker on the Legacy DICOM Gateway. At most sites Station Number equals the Internal Entry Number (IEN), but not all are like Orlando. The DICOM TRANSMIT DESTINATION file #2006.587 has a pointer to the INSTITUTION file (#4) IEN and the STATNUMB^MAGDFCNV returns the actual Station Number. Resolution: ----------- The comparison in ^MAGDSTQ9 was changed to convert the IEN to the Station Number to do the proper comparison. 9. Defect IMAG-3046 - Automatic Retrieve is not finding studies with the Accession Number Problem: -------- When running the RET Retrieve Missing Images option for both Radiology and Consults, the query is not finding the study with the accession number and then defaults to using the Study Instance UID. The Study Instance UID does not always return all the images. Patch MAG*3.0*306 changed the accession number query by adding the Last 4 digits of the medical record number (SSN) as a Patient ID (0010,0020) wild-card query key (that is "*nnnn"). While this technique worked for the VISN PACS that had the problem, it caused difficulties with McKesson PACS elsewhere which rejected queries using wild-card Patient ID keys. Resolution: ----------- An option was added to the Study Tracker PARM Set Query/Retrieve Site Parameters to enable or disable the use of the Patient ID wild-card query key. 10. IMAG-2599 - (INC19580178) and IMAG-1982 (INC17998073) - DICOM Gateway Fatal Error Problem: -------- The DICOM Gateway sometimes receives an unexpected value at a specific place in the process. It expects '04H' but if that is not the value read, then an entry is made in the DICOM LOG and that entry is also emailed to the contact stored in the configuration. The message does not have the IP address of the source system so there is no way for the site to identify the server sending the bad data. Resolution: ----------- Routine MAGDTCP3 will be modified in subroutine RECVDATA to log an error in the Cache error trap with the following error text identifying the IP address: "Unexpected PDU in from IP address: " The log and email message will be updated to say "Check error trap for more information" 11. IMAG-3422 - Routing Evaluation Processor would not start Problem: -------- When testing MAG*3.0*305 T2, the Routing Evaluation Processor would not start and the error message was causing an error at SEND+2^MAGBRTUT was being displayed. Resolution: ----------- Corrected the RPC to set the return variable in all situations. 12. IMAG-3423 - When stopping the Routing Evaluation Processor would look like an error Problem: -------- During testing MAG*3.0*305 T2 when stopping the Routing Evaluation Processor the message Remote process has ended: -3,Inactive: Finished was displayed which looked like an error message but was not. Resolution: ----------- Updated the display when stopping the Evaluation Processor to only Display Remote process has ended. 13. IMAG-3424 - Could not select a patient in batch exports when multiple patients displayed Problem: -------- When running a Batch Export in the LDGW and only a partial name was entered, a list of patients that matched are displayed. After selecting a patient from the list it would keep going back to the list and not allow a patient to be selected. Resolution: ----------- Corrected the selection of the patient so it would not keep looping. 14. IMAG-3425 - Processing error on 2-8-2 window Problem: -------- The export was displaying an error on certain Targa images being processed. 30 Bad Targa File -- \\VHAHAMIMMCLU2A.V06.DOMAIN.EXT\IMAGE8$\HAM0\00\ 70\72\36\HAM00070723631.DCM Neither RGB color, nor gray scale. Image Type is " 0". Resolution: ----------- Modified the export to handle situations where both Targa and DICOM files are created for an image. All sites running VistA Imaging 3.0 must install the KIDS portion of this patch on the VistA System. This patch can be loaded while the VistA Imaging System is active, and users are on the system (please see note below regarding 2-8-2 processing). Installing the MAG*3.0*305 KIDS takes 2-5 minutes. Note: This must be a coordinated install with the VistA Imaging team due to possible impact to 2-8-2 processing on the Gateway(s). Previous versions of the 2-8-2 process will no longer run after MAG*3.0*305 KIDS is installed. This patch must be installed by the compliance date to conform to VHA Directive 2001-023. Note: The MAG*3.0*305 KIDS should be installed on VistA first and then the DICOM Gateway updated. The VistA DICOM Gateway query/retrieve capabilities require DICOM Gateway services and will not be operational until MAG*3.0*305 is installed on the DICOM Gateways. Following the KIDS install, the DICOM Gateways can be updated. See Below. Installation Instructions: -------------------------- The actual KIDS installation will take less than 2 minutes. 1 On the VistA system, access the Kernel Installation and Distribution System Menu [XPD MAIN]. 2 Run the Installation option [XPD INSTALLATION MENU]. 3 Load the KIDS file by performing the following steps: a. Run the Load a Distribution option [XPD LOAD DISTRIBUTION] to load the KIDS distribution. b. When prompted, enter the path and file name (MAG3_0P305.KID) of the Patch 305 KIDS file. c. When prompted to continue with the load, enter "YES". A Distribution OK! message will be displayed when the load is complete. 4 After loading the KIDS file, use the following options to verify the contents of the patch and to back up any affected routines a. Verify Checksums in Transport Global [XPD PRINT CHECKSUM] - Run this option to ensure the integrity of the routines in the patch. b. Compare Transport Global to Current System [XPD COMPARE TO SYSTEM] - Run this option to view all changes that will be made when the patch is installed. All components (routines, options, and so on) in the patch will be compared. c. Backup a Transport Global [XPD BACKUP] - Run this option to create a backup message of any Routines and the Build for this patch. 5 After performing the load and any optional verification steps, install the KIDS file by performing the following steps: a. Run the Install Package(s) [XPD INSTALL BUILD] option. b. When prompted for the install name, enter "MAG*3.0*305". c. Answer "NO" to the following prompts: Want KIDS to Rebuild Menu Trees Upon Completion of Install? NO// Want KIDS to INHIBIT LOGONs during the install? NO// Want to DISABLE Scheduled Options, Menu Options, and Protocols? NO// 6 When installation is finished, an Install Complete message will be displayed. Post-Installation Instructions: ------------------------------- ALL DICOM Gateways, including Legacy, Text and Routing, as well as HDIGs, need to be upgraded with the installation of MAG3_0P305_DICOM_Setup.exe at your earliest convenience. Refer to the VistA Imaging DICOM Gateway Installation Guide for details. Note: Backup the DICOM dictionary directory. Typically, the DICOM Dictionaries are installed in the \DICOM\Dict directory on a network drive. IMPORTANT: The Cache Service must be stopped before upgrading the DICOM Gateway. Note: If Caché is installed during the installation, it will automatically install Adobe SVG Viewer 3.0 which is not approved for use and must therefore be uninstalled. Please check if Adobe SVG Viewer 3.0 was installed under Add/Remove programs and uninstall if necessary. Note: If installing the LDGW on a new server, the latest 2008 C++ Redistributables must be downloaded and installed prior to running the install of the LDGW. Refer to the VistA Imaging DICOM Gateway Installation Guide for details. (Section 4.7.3) Back-Out/Roll Back Plan: ------------------------ Uninstalling the Application: If it is necessary to uninstall the MAG*3.0*305, then the user would follow the following steps: 1. Stop Cache 2014 by selecting the blue Caché cube and select Stop Cache. 2. Copy the file Cache.key in the C:\CacheSys\mgr directory to a temporary location. 3. Go to the Control Panel, choose Uninstall a program, and remove the current version Cache instance. 4. Go to the Control Panel, choose Uninstall a program, and remove the VistA DICOM Gateway (Patch 305). 5. Delete the directory C:\CacheSys. 6. Restart the server. 7. Install Legacy DICOM Gateway patch MAG*3.0*274, refer to the VistA Imaging DICOM Gateway Installation Guide for instructions. 8. Copy the Cache.key file backed up earlier to C:\CacheSys\mgr. 9. Restore the DICOM dictionary directory from the backup made during installation. KIDS Uninstall: If it is necessary to uninstall the MAG*3.0*305 VistA KIDS, then select the "Kernel Installation & Distribution System" menu option, "Backup a Transport Global" (see Installation Steps section, step 4c must be done before installing the patch). Administrators will need to use the PackMan function INSTALL/CHECK MESSAGE. Check the MailMan messages for the backup message sent by the "Backup a Transport Global" function executed prior to the patch install. 1.
