As part of this initiative, we will collect and/or extract seven additional data items. The following data items will be derived from the GMRV VITAL MEASUREMENT (#120.5) file. HEIGHT (#160,1011) WEIGHT (#160,1012) The following data items will be prompted for in the Comorbidity/ Complication and Patient History portion of the abstract. If GRADE/DIFFERENTIATION was 9 (Unknown/Not determined, stated or applicable), automatic staging was calculating stage group values of 99 (Unknown). This was incorrect and has been fixed. 4) 7th Edition staging of Prostate GLEASON SCORE (CLINICAL) (#165.5,623) GLEASON SCORE (PATHOLOGIC) (#165.5,250) 7th Edition staging for Prostate requires Gleason Score for stage group calculation. Automatic staging was using the same Gleason Score value for computing both STAGE GROUP CLINICAL (#165.5,38) and STAGE GROUP PATHOLOGIC (#165.5,88). This was incorrect. GLEASON'S SCORE has been renamed GLEASON SCORE (CLINICAL) and a new data item, GLEASON SCORE (PATHOLOGIC) has been added. STAGE GROUP CLINICAL will now use GLEASON SCORE (CLINICAL) and STAGE GROUP PATHOLOGIC will use GLEASON SCORE (PATHOLOGIC) to compute stage group. 5) [AI Complete Abstract] Existing primary display The display which shows a patient's existing primaries via the [AI Complete Abstract] option has been expanded to include DATE LAST CONTACT (#160,16), STATUS (#160,15) and FOLLOW-UP STATUS (#15.2). The display now looks as follows: Acc/Sequence Primary Site Last Tumor Status Date Dx Stat Div ------------- -------------------- ----------------- ---------- ---- ---- 2011-00001/00 BONE MARROW Evid this tumor 01/01/2011 Inc 528 2011-00010/00 NERVES, HEAD & NECK Evid this tumor 06/23/2011 Inc 528 Date Last Contact: 11/03/2011 <--new Status: Alive <--new Follow-up Status: Active <--new 6) NAACCR (North American Association of Central Cancer Registries) v12.2 This patch implements NAACCR Record Layout v12.2. The following changes have been included in this patch: FIN Coding System [35] 3-3 has been retired. 7) [PF Post/Edit Follow-up] DATE CASE LAST CHANGED (#165.5,198) When a new follow-up is added the program sets the DATE CASE LAST CHANGED value for all of the patient's 'Complete' cases to the current date. However, the cross-reference associated with DATE CASE LAST CHANGED was not being updated. This failure to update the cross-reference would cause "changed" cases from not being included in NCDB, State and VACCR extracts for a specified DATE CASE LAST CHANGED range. This has been fixed and the DATE CASE LAST CHANGED cross-reference has been re-indexed. 8) CLIN TNM DOCUMENTATION PRE-TX (#165.5,280) CL TNM DOCUMENTATION LOCATION (#165.5,280.1) CL TNM DOCUMENTATION DATE (#165.5,280.2) TX GUIDELINES DISCUSSION (#165.5,281) CLINICAL TRIALS DISCUSSION (#165.5,279) The following new data items have been added to the abstract input dialog: CLIN TNM DOCUMENTATION PRE-TX (#165.5,280) CL TNM DOCUMENTATION LOCATION (#165.5,280.1) CL TNM DOCUMENTATION DATE (#165.5,280.2) These three data items appears in the "Stage of Disease at Diagnosis" section immediately after STAGED BY (CLINICAL STAGE) (#165.5,19). TX GUIDELINES DISCUSSION This data item appears in the "First Course of Treatment" section after CAP TEXT (#165.5,47). CLINICAL TRIALS DISCUSSION This data item appears in the "First Course of Treatment" section after PALLIATIVE CARE (#165.5,12). 9) "Name--First" [2240] 3380-3419 The NAACCR (North American Association of Central Cancer Registries) data item "Name--First" is derived from the NAME (#.01) field of the PATIENT (#2) file. If NAME consisted of a single name (no given or first name), the program would extract a value for "Name--First". This would cause the following EDITS error: E: is not a valid value for Name--First This has been fixed. If the derived "Name--First" value is , the program will derive a value of UNKNOWN. 10) 5th and 6th Edition staging of Breast If the PATHOLOGIC T (#165.5,85) value of "T1mic" was selected the registrar would get the following EDITS error: Edit: TNM Path T (COC) E: 1MIC is not a valid value for TNM Path T "T1mic" has been replaced with the valid value "T1mi". 11) [DF Print Due Follow-up List by Month Due] [LF Print Delinquent (LTF) List] The [DF Print Due Follow-up List by Month Due] and [LF Print Delinquent (LTF) List] "Standard format" reports have been modified to display the full Social Security Number (rather than just the PID#). 12) ABSTRACTED BY (#165.5,92) "Abstracted By" [570] 742-744 ABSTRACTED BY contains the abstracter's full name as it appears in the NEW PERSON (#200) file. "Abstracted By" [570] 742-744 in the NAACCR (North American Association of Central Cancer Registries) extract record is 3 characters in length and contains the abstracter's initials as they appear in the INITIAL (#200,1) field of the NEW PERSON file. If INITIAL is blank in the NEW PERSON file the following EDITS error would occur: Abstracted By, Date of Diagnosis (COC) E:Abstracted By cannot be blank if Date of Diagnosis is greater than 1995 Abstracted By (742) = Date of Diagnosis (530) = Y:2009 M:09 D:04 To resolve this error, if INITIAL is blank, the NAACCR extract logic will now extract an "Abstracted By" value of 999. 13) [CF Automatic Casefinding-Lab Search] In patch ONC*2.11*53 Lab casefinding was enhanced to capture the following cases: Squamous cell neoplasms (805-808) of the skin Basal cell neoplasms (809) Some sites would prefer not to capture these cases. Therefore, squamous and basal cell neoplasm capture has been made optional. The registrars will now be asked the following question: Include Squamous and Basal cell neoplasms? Yes// Answering "Yes" will include squamous and basal cell neoplasms. Answering "No" will exclude them. 14) [QA Print Abstract QA (80c)] a) The following data items have been added to the CANCER REGISTRY QA ABSTRACT. ELAPSED MONTHS TO COMPLETION (#165.5,157.1) ACCESSION NUMBER (#165.5,.05) SEQUENCE NUMBER (#165.5,.06) CANCER REGISTRY QA ABSTRACT OCT 26,2011 Abstracted by.................: Registrar,Test Reporting Facility............: Test Va Medical Center Date Case Completed...........: 08/09/2001 Elapsed Months to Completion..: 19 <--added Patient Name..................: PATIENT,TEST PID#..........................: C4444 Accession Number..............: 201100003 <--added Sequence Number...............: 01 <--added a) The registrar will now see the following prompt: Exclude sensitive patient data? No// If the registrar answers "YES", the following data item values will be masked from display: PATIENT NAME PID# 15) New FACILITY FILE (#160.19) entry The following entry has been added to the FACILITY file: 10001223 VA TEXAS VALLEY COASTAL BEND HCS NPI: 1548588841 16) Collaborative Stage version 0204 This patch implements CS version 0204. The necessary data conversions will be performed when the patch is installed. This patch also identifies cases for which CS 0204 review/recoding is required, recommended or suggested. The data items CS REVIEW REQUIRED (#165.5,247) and CS FIELD NEEDING REVIEW (#165.5,283) will identify the cases and data items needing review. If, during the CS 0204 conversion process, a case satisfies the "review/recode" criteria, CS REVIEW REQUIRED will be set to 1 (Needs review) and CS FIELD NEEDING REVIEW will list the field(s) needing review. The registrar may then create a list of cases requiring review/recoding via the [CPR Print Custom Reports] or [DS Define Search Criteria] options by sorting or searching for CS REVIEW REQUIRED values of "Needs review". Upon reviewing the case, the registrar can change CS REVIEW REQUIRED to 0 (Reviewed) thereby removing the case from any future review/recode lists. The [CS Restage CS cases using latest version] option has also been enhanced to include error messages when restaging error or warnings are encountered. e.g. Restaging using CS Version 02.04.99 for UPSTATE NEW YORK HCS K5456 C44.5 200100003/13 encountered a CS warning Extension lookup of code 800 failed. <--error message added 17) 7th Edition staging of Mucosal Melanoma of the Head and Neck 7th Edition cases for Mucosal Melanoma of the Head and Neck have a TNM staging schema. These cases were being stuffed with "no staging" values. This was incorrect. These cases may now be staged. 18) TREATMENT ABBREVIATED (#165.5,42) For cases in which all of the treatments were either "None" and/or "Unknown", the TREATMENT ABBREVIATED value was being computed as "NONE". The computed TREATMENT ABBREVIATED value for these cases has been changed to "UNKNOWN". 19) CLINICAL N (#165.5,37.2), CLINICAL M (#165.5,37.3) and PATHOLOGIC N (#165.5, 86) fields expanded For Breast cases there were some field values which were added in 7th Edition Staging which were too long to be entered into the existing fields. Examples include M0(i+), N0(MOL-) and N0(MOL+). The field lengths were expanded so that these values may now be entered correctly. Also the Pathologic M (#165.5, 87) field was giving incorrect help text due to a bug in the Help Text code. This issue was also corrected so that Pathologic M help text displays correctly. 20) ABSTRACT STATUS (#165.5,91) field reset to Incomplete if required fields are deleted from a Completed case If a user deletes any required fields from a case with an Abstract Status of Complete, the software will now re-check the required fields and display a message to the user that the Abstract Status is being changed back to Incomplete. Before this change the software was inadvertently allowing the case to remain Complete.
