INC17067067 VISTA Inpatient Pharmacy error with clinic orders and stop dates Associated NSR(s): ================== N/A Participating Test Sites: ========================= VA Pittsburgh HCS, Pittsburgh PA VA Gulf Coast VHCS, Biloxi MS Patient Safety Issues (PSIs): ============================= HITPS-7828 Defect Tracking System Ticket(s) & Overview: =========================================== 1. INC17067067 VISTA Inpatient Pharmacy error with clinic orders and stop dates Problem: ------- The issue is seen with "outpatients" and unit dose clinic medications. The function to specify number of doses in the stop date field is calculating incorrectly; it is one dose short. When entering a dose limit for an outpatient unit dose clinic medication (for an outpatient who has never been admitted), the stop date calculated is at the date/time of the last dose to be administered. As a result, in BCMA that last dose cannot be administered at the time the order is defined to stop. To fix this, the stop date/time should be calculated to be after the last dose to be administered. Resolution: ---------- Modify routines PSGDL & PSJORP2 so that the date/time of the first dose is used to determine the stop date/time using the dose limit entered. Prior to this patch the outpatient clinic order was using the current time (for an outpatient who has never been admitted) to calculate the dose limit entered to get the stop date/time which would come up with one dose less than the dose limit entered. This patch modifies the code so that the dose limit calculation for all clinic medications have the same calculation logic as if inpatient medication orders would have when INPATIENT WARD PARAMETERS (File #59.6) field DEFAULT START DATE CALCULATION (field #.05) is set to 2 ("USE NOW AS DEFAULT"). To further clarify, the dose limit calculation for clinic medications will function the same regardless of the value in the DEFAULT START DATE CALCULATION parameter.
