Patient Safety Issues (PSIs): ---------------------------- HITPS-8683 Defect Tracking System Ticket(s) & Overview: =========================================== INC22313768: Incorrect Coflex Allergy Interpretation Problem: ------- When an allergy is entered to a Coflex product, the allergy is incorrectly interpreted as being to "orphenadrine". (Coflex is a bandage supply item, and orphenadrine is a prescription medication.) Resolution: ---------- Post-install routine PSN574P will: 1. In the NDC/UPN (#50.67) file, search for entries which have the VA PRODUCT NAME (#5) field defined for "ORPHENADRINE CITRATE" and the TRADE NAME (#4) field defined for "COFLEX". Replace "COFLEX" with "ORPHENADRINE CITRATE". 2. In the PATIENT ALLERGIES (#120.8) file, search for entries which have the GMR ALLERGIES (#1) field defined for "ORPHENADRINE CITRATE" and the REACTANT (#.02) field defined for "COFLEX". Replace "COFLEX" with "ORPHENADRINE CITRATE". It was decided that this is the appropriate modification instead of replacing the GMR ALLERGIES (#1) field with a "bandage" entry. The patient may, in fact, have an allergy to Orphenadrine Citrate. Since the allergy is already filed for the patient, healthcare personnel would not file a duplicate allergy entry. For that reason, the MailMan message will state that the charts for the listed patients should have their charts checked to determine if the allergy exists. 3. Send a MailMan message to the installer as well as holders of the PSNMGR security key.
