A patient safety ticket has been logged because providers who use the CPRS Orders Tab to review medication orders will be under the impression that a patient has an ongoing medication order when in fact the order has expired or has been discontinued. Medication orders with this issue display correctly under the CPRS Meds tab. *************************PLEASE NOTE***************************** Many of the orders found with this issue are very old, from 1998-1999, so reports should be run back to 1/1/1998. ***************************************************************** Patch Components: ================= Options Associated: Option Name Type New/Modified/Deleted ----------- ----------- -------------------- PSO MEDICATION STATUS CHECK run routine New PSO SUPERVISOR menu Modified New Service Requests (NSRs): =========================== N/A Patient Safety Issues (PSIs): ============================ HITPS-6374 Associated ticket(s): ===================== INC1684422 - Medication File Status Mismatches INC2322377 (d) INC3194336 (d) Defect Tracking System Ticket(s) & Overview: =========================================== INC1684422: Orders File / Medication File Status Mismatches Issue: ------ Discontinued or expired medication orders might display in CPRS with an "active" status under the Orders tab instead of the correct "discontinued" or "expired" status. A patient safety ticket has been logged because providers who use the CPRS Orders Tab to review medication orders will be under the impression that a patient has an ongoing medication order when in fact the order has expired or has been discontinued. Medication orders with this issue display correctly under the CPRS Meds tab. Resolution: ---------- Create option Medication Status Check [PSO MEDICATION STATUS CHECK], and add to the Supervisor Functions [PSO SUPERVISOR] menu. Outpatient Pharmacy Manager [PSO MANAGER] Supervisor Functions [PSO SUPERVISOR] Medication Status Check [PSO MEDICATION STATUS CHECK] The option will allow date ranges of the ORDERS (#100) file to be searched. The user is also asked which types of orders should be searched: I Inpatient O Outpatient N Non-VA IN Inpatient and Non-VA ON Outpatient and Non-VA A All "Inpatient" and "Outpatient" refer to the order dialog used, not the patient's status. The search option asks the user if the affected orders should have an erroneously active ORDERS (#100) file status field automatically corrected to discontinued or expired while searching. Sites may want to first determine how widespread the issue has occurred before deciding whether to correct the issue automatically or manually. Most of the orders found with this issue are very old, from 1998-1999, so reports should be run back to 1/1/1998. After the user verifies selection criteria, the option is tasked to run in the background. The status of the ORDERS (#100) file entry is compared to the associated medication file entry. Medication file entries will either be in the PRESCRIPTION (#52) file for outpatients or the PHARMACY PATIENT (#55) file for inpatient medications and non-VA medications. Non-verified orders are not checked. If a status mismatch exists between the ORDERS (#100) file and the associated medication file, information such as order and prescription numbers is sent to the user in a MailMan message. Instructions to check the reported orders are included. If no status mismatches exist for the selection criteria, the MailMan message will state that there are no issues. An alert is sent to the user after the option completes so that the user will know that a MailMan message is now available.
