The logic excluded drugs if it contained the term 'BUPREN' and was a PATCH or FILM,BUCCAL dosage form, such as Butrans (patch) and Belbuca (film, buccal). This enhancement replaces and reimplements the functionality introduced by patches PSS*1.0*219 and PSS*1.0*246. It introduces a new multiple valued XPAR parameter to hold all Buprenorphine products that are NOT considered detox drugs based on VA PRODUCT file (#50.68), which are products that are FDA approved with an indication for the treatment of pain. Any medication that is matched to any of the new parameter entries will not require the prescribing provider who signs the order to have an X-number (Detox Number). Otherwise, if the medication is a detox medication then the provider should have the Detox number defined in his/her record. This patch exports a new menu option to replace an existing one under the Dosages [PSS DOSAGES MANAGEMENT] menu as follows: - New option: Manage Buprenorphine Tx of Pain - VA Products [PSS BUPRENORPHINE VAPRODS] - Old option: Manage Buprenorphine Tx of Pain Dosage Forms [PSS BUPRENORPHINE DOSAGE FORMS] The new option allows users to manage the contents of the new parameter if new products are added in the future. This field may be set to "VAMC", for traditional VA Medical Center pharmacy mode, or "MBM", for Meds By Mail pharmacy mode. Only the Meds By Mail Pharmacy should set this parameter to "MBM". If this parameter is left empty, it will default to "VAMC". Functionality is being released in patch PSO*7.0*630 that uses this parameter to help determine if a provider's DEA related information may be edited at the facility. Pre-Installation Instructions: This patch may be installed with users on the system although it is recommended that it be installed during non-peak hours to minimize potential disruption to users. This patch should take less than 5 minutes to install. Installation Instructions: 1. Choose the PackMan message containing this build. Then select the INSTALL/CHECK MESSAGE PackMan option to load the build. 2. From the Kernel Installation and Distribution System Menu, select the Installation Menu. From this menu, A. Select the Verify Checksums in Transport Global option to confirm the integrity of the routines that are in the transport global. When prompted for the INSTALL NAME enter the patch or build name. (ex. PSS*1.0*247) B. Select the Backup a Transport Global option to create a backup message. You must use this option and specify what to backup; the entire Build or just Routines. The backup message can be used to restore the routines and components of the build to the pre-patch condition. i. At the Installation option menu, select Backup a Transport Global ii. At the Select INSTALL NAME prompt, enter your build PSS*1.0*247 iii. When prompted for the following, enter "R" for Routines or "B" for Build. Select one of the following: B Build R Routines Enter response: Build iv. When prompted "Do you wish to secure your build? NO//", press and take the default response of "NO". v. When prompted with, "Send mail to: Last name, First Name", press to take default recipient. Add any additional recipients. vi. When prompted with "Select basket to send to: IN//", press and take the default IN mailbox or select a different mailbox. C. You may also elect to use the following options: i. Print Transport Global - This option will allow you to view the components of the KIDS build. ii. Compare Transport Global to Current System - This option will allow you to view all changes that will be made when this patch is installed. It compares all of the components of this patch, such as routines, DDs, templates, etc. D. Select the Install Package(s) option and choose the patch to install. i. If prompted 'Want KIDS to Rebuild Menu Trees Upon Completion of Install? NO//', answer . ii. When prompted 'Want KIDS to INHIBIT LOGONs during the install? NO//', answer . iii. When prompted 'Want to DISABLE Scheduled Options, Menu Options, and Protocols? NO//', answer . Post Installation Instructions: ------------------------------- None. Installation Verification: -------------------------- Successful installation can be verified by reviewing the first 2 lines of the routines contained in the patch. The successful installation of the Data Dictionary may be verified by using FileMan. The following steps may be performed: 1. Select FileMan DATA DICTIONARY UTILITIES option. 2. At the prompt 'Select DATA DICTIONARY UTLITY OPTION:', enter LIST FILE ATTRIBUTES. 3. At the prompt 'START WITH What File:' enter PHARMACY SYSTEM. 4. At the prompt 'GO TO What File:', accept the default (PHARMACY SYSTEM). 5. At the prompt 'Select SUB-FILE:' accept the default (blank). 6. At the prompt 'Select LISTING FORMAT:' accept the default (STANDARD). 7. At the prompt 'Start with field:', enter 102. Verify the text PHARMACY OPEARING MODE displays immediately after the entry of 102. 8. At the prompt 'Go to field:' enter 102. 9. At the prompt 'DEVICE:', print to the screen or other available device. 10. Verify the output shows the PHARMACY OPERATING MODE field is stored at global location '0;2' and consists of a set of codes with two possible values, 'MBM' and 'VAMC'. Back-out/Rollback Strategy: --------------------------- Back-out will be done only with the concurrence and participation of development team and appropriate VA site/region personnel. The decision to back-out or rollback software will be a joint decision between development team, VA site/region personnel and other appropriate VA personnel. Prior to installing an updated KIDS package, the site/region should have saved a backup of the routines in a mail message using the Backup a Transport Global [XPD BACKUP] menu option (this is done at time of install). The message containing the backed-up routines can be loaded with the "Xtract PackMan" function at the Message Action prompt. The Packman function "INSTALL/CHECK MESSAGE" is then used to install the backed-up routines onto the VistA System. The removal of the Data Dictionary must be done manually using the FileMan MODIFY FILE ATTRIBUTES option. The following steps must be performed: 1. Select the FileMan option MODIFY FILE ATTRIBUTES. 2. At the prompt 'Do you want to use the screen-mode version?' enter NO. 3. At the prompt 'Modify what File:', enter PHARMACY SYSTEM or 59.7. 4. At the prompt 'Select FIELD:' enter PHARMACY OPERATING MODE or 102. 5. At the prompt 'LABEL: PHARMACY OPERATING MODE Replace', enter @. 6. At the prompt 'SURE YOU WANT TO DELETE THE ENTIRE 'PHARMACY OPERATING MODE' FIELD?', enter Y. 7. At the prompt 'OK TO DELETE 'PHARMACY OPERATING MODE' FIELDS IN THE EXISTING ENTRIES?', enter Y. After backing out this patch, sites will need to use restored option Manage Buprenorphine Tx of Pain Dosage Forms [PSS BUPRENORPHINE DOSAGE FORMS] to repopulate the dosage form exception list at the Package level. Users need elevated (programmer) privileges to edit at the Package level. - FILM,BUCCAL - PATCH Since data was not modified by the patch installation, a rollback strategy is not required. Validation of Back-out Procedure --------------------------------- The Back-out Procedure can be verified by printing the first 2 lines of the PSS Routines contained in this patch using the option First Line Routine Print [XU FIRST LINE PRINT]. Once the routines contained in the PSS*1.0*247 patch have been rolled back, the first two lines of the Routines will no longer contain the designation of patch PSS*1.0*247 in the patch list section on line 2. The successful backout of the Data Dictionary may be verified by using FileMan. The following steps may be performed: 1. Select FileMan DATA DICTIONARY UTILITIES option. 2. At the prompt 'Select DATA DICTIONARY UTLITY OPTION:', enter LIST FILE ATTRIBUTES. 3. At the prompt 'START WITH What File:' enter PHARMACY SYSTEM. 4. At the prompt 'GO TO What File:', accept the default (PHARMACY SYSTEM). 5. At the prompt 'Select SUB-FILE:' accept the default (blank). 6. At the prompt 'Select LISTING FORMAT:' accept the default (STANDARD). 7. At the prompt 'Start with field:', enter 102. Verify two question marks (??) display immediately after the entry of 102. This indicates the field has been successfully removed.
