Problem: -------- The NRPO requests updates to radiology mammography reports so they include the facility name, city, state, zip code and phone number of the facility with the report header, to comply with the new FDA requirement. Resolution: ----------- VistA radiology report options and the CPRS radiology reports have been updated to include the Official Facility Name, City, State, Zip code, and main phone number with the report header information. The new information will display on ALL locally performed radiology exams, but not on outside/community care exams (those should include the information for the outside facility where they were performed). Technical Resolution: --------------------- The facility name and address information are pulled from the INSTITUTION (#4) file, but the facility phone number is not available there. A new FACILITY PHONE NUMBER (#200) field is added to the RAD/NUC MED DIVISION (#79) file, that will need to be populated by the site personnel. The Division Parameter Set-up [RA SYSDIV] option is modified for the new FACILITY PHONE NUMBER (#200) field. The field can be edited independently or together with the rest of the division parameters. The Print Division Parameter List [RA SYSDIVLIST] option is also modified to display the facility phone number, if entered. is displayed if the information has not been entered. This patch should take less than 5 minutes to install. The following menus need to be disabled during installation for this patch. During installation, when prompted 'Want to DISABLE Scheduled Options, Menu Options, and Protocols? YES//', answer YES, then you can enter these options at the prompt. Choose the PackMan message containing this build. Then select the INSTALL/CHECK MESSAGE PackMan option to load the build. 2. From the Kernel Installation and Distribution System Menu, select the Installation Menu. From this menu, A. Select the Verify Checksums in Transport Global option to confirm the integrity of the routines that are in the transport global. When prompted for the INSTALL NAME enter RA*5.0*210 B. Select the Backup a Transport Global option to create a backup message. You must use this option and specify what to backup; the entire Build or just Routines. The backup message can be used to restore the routines and components of the build to the pre-patch condition. i. At the Installation option menu, select Backup a Transport Global ii. At the Select INSTALL NAME prompt, enter your build RA*5.0*210. iii. When prompted for the following, enter "R" for Routines or "B" for Build. Select one of the following: B Build R Routines Enter response: Build iv. When prompted "Do you wish to secure this message? NO//", press and take the default response of "NO". v. When prompted with, "Send mail to: Last name, First Name", press to take default recipient. Add any additional recipients. vi. When prompted with "Select basket to send to: IN//", press and take the default IN mailbox or select a different mailbox. C. You may also elect to use the following options: i. Print Transport Global - This option will allow you to view the components of the KIDS build. ii. Compare Transport Global to Current System - This option will allow you to view all changes that will be made when this patch is installed. It compares all of the components of this patch, such as routines, DDs, templates, etc. D. Select the Install Package(s) option and choose the patch to install. i. If prompted 'Want KIDS to Rebuild Menu Trees Upon Completion of Install? NO//', answer NO ii. When prompted 'Want KIDS to INHIBIT LOGONs during the install? NO//', answer NO. iii. When prompted 'Want to DISABLE Scheduled Options, Menu Options, and Protocols? YES//', answer YES. Instructions for ADPAC to add the Facility Phone Number to a Radiology Division: 1. Navigate to the Division Parameter Set-up [RA SYSDIV] option on the System Definition Menu [RA SYSDEF]. 2. Select a division. 3. You will see a new prompt: "Would you like to enter/edit the FACILITY PHONE NUMBER only? NO//", answer YES. *Note that answering "NO" will let you edit all division parameters like before, including the new phone number field. 4. Enter the main phone number for the facility associated with that division in the format nnn-nnn-nnnn or (nnn) nnn-nnnn. No service phone extensions can be entered. 5. Use the Print Division Parameter List [RA SYSDIVLIST] option to print the radiology division parameters. Confirm the phone number you entered is listed. Divisions that don't have a phone number entered will display Back-out/Rollback Strategy: --------------------------- Back-out will be done only with the concurrence and participation of development team and appropriate VA site/region personnel. The decision to back-out or rollback software will be a joint decision between development team, VA site/region personnel and other appropriate VA personnel. A national help desk ticket should be logged before attempting any backout procedures. Prior to installing an updated KIDS package, the site/region should have saved a backup of the routines in a mail message using the Backup a Transport Global [XPD BACKUP] menu option (this is done at time of install). The message containing the backed up routines can be loaded with the "Xtract PackMan" function at the Message Action prompt. The Packman function "INSTALL/CHECK MESSAGE" is then used to install the backed up routines onto the VistA System. If the new backup build option was used, after Xtracting the patch message use the KIDS Installation menu and enter RA*5.0*210b at the INSTALL NAME prompt.
