 This patch will update the DOCCNT^TIUSRVLV API (ICR #2812). It will add 
 another parameter (CNTSCND), that when set to 1, will also count documents
 that reference the Visit via the SECONDARY VISIT field. It is backwards 
 compatible, as this parameter is optional and will default to 0 (which was
 the previous behavior).
