Clinical Institute Withdrawal Assessment - Alcohol Revised CIWA-AR- -- This instrument is modified to include the missing text and displays the information in the correct order. This instrument replaces CIWA-AR. Katz Index of Independence in Activities of Daily Living KATZ-ADL-18PT -- Prior to this patch, this instrument name was INDEX OF ADL. Now it is KATZ-ADL-18PT. Patient Health Questionnaire Depression Scale PHQ9 -- The report was updated to include additional text beneath the score to indicate additional assessment is required if question 9 is answered in the positive direction. Working Alliance Inventory-Short Revised WAI-SR -- A scoring error has been corrected. This patch also changes the cut off point for the Posttraumatic Stress Disorder Checklist 5 (PCL-5) instrument from 38 to 33. This affects the text that is displayed with using PCL-5 from a reminder dialog. Recent PCL-5 results are now included in the PTSD reminder dialogs. The reports for Addiction Severity Index (ASI) and Seclusion/Restraints have been updated to use the last 4 of the SSN instead of the full SSN. A new option has been added to the YTQ MHA3 MENU called Instrument Exchange. This allow updated instrument definitions to be loaded from a file or web site and viewed or installed.
