 1) VAC-0303-20793  ADD C&P AMIE WORKSHEET
  
    Problem:
    -------
    1) Dr. Tomlinson (C&P Regulation Office, VACO) requested that a new
       attachment be added to the COLD INJURY PROTOCOL EXAMINATION
       worksheet (#1730).  It would be named ATTACHMENT B:  PROTOCOL
       EXAMINATION HISTORY FOR COLD INJURIES. 
        
    Resolution: 
    ----------
    1) Routine DVBCWCI2 was created to contain the text of the new
       ATTACHMENT B examination sheet.
    2) Created routine DVBCWCI3 to print the text contained in DVBCWCI2.
    3) Created routine DVBCWCI4 from DVBCWCI. DVBCWCI4 will print
       the current worksheet #1730 and then call DVBCWCI3 to print the
       new attachment B worksheet.
    4) Created post-init routine DVBA52PI to deactivate DVBCWCI and
       activate DVBCWCI4 as the REPORTING PROGRAM NAME (field #7) for
       the COLD INJURY PROTOCOL EXAMINATION (#147) entry in the AMIE EXAM
       (#396.6) file.

    To view the format of the new ATTACHMENT B, use the following options
    to print a blank C&P worksheet.
       
       Options:

  1.  Choose the PackMan message containing this patch and invoke the 
      INSTALL/CHECK MESSAGE PackMan option.
  
  2.  Start up the Kernel Installation and Distribution System Menu 
      [XPD MAIN]: 
    
           Edits and Distribution ...  
           Utilities ...  
           Installation ...  
    
      Select Kernel Installation & Distribution System Option: INStallation

           Compare Transport Global to Current System

  3.  From this menu, you may elect to use the following options
      (when prompted for the INSTALL NAME, enter DVBA*2.7*52):
  
      a.  Compare Transport Global to Current System - This option will
          allow you to view all changes that will be made when this patch
          is installed.  It compares all components of this patch
          (routines, DD's, templates, etc.).
      b.  Verify Checksums in Transport Global - This option will allow
          you to ensure the integrity of the routines that are in the
          transport global.
  
  4.  Use the Install Package(s) option and select the package DVBA*2.7*52.
  
      a.  When prompted 'Want KIDS to INHIBIT LOGONs during the install?
          YES//', answer NO (unless otherwise indicated).
      b.  When prompted 'Want to DISABLE Scheduled Options, Menu Options,
          and Protocols? YES//', answer YES.
      c.  When prompted 'Enter options you wish to mark as 'Out Of
          Order':', enter the following options:
  
            Regional Office C&P Menu        [DVBA C REGIONAL OFFICE MENU]
            Medical Administration C&P Menu [DVBA C MEDICAL ADMIN C&P MENU]
   
      d.  When prompted 'Enter protocols you wish to mark as 'Out Of
          Order':', press <return>.
  
  5.  (DSM for Open VMS sites only) Optional - Include the routines
      distributed with this patch in the mapped routine set.
     
      NOTE: This step is only necessary if you wish to include the routines 
      in your mapped set.
  
  6.  Optional - After successful installation of this patch, you may
      delete the post-init routine DVBA52PI.
