MDC 8 (p.47303) (a) New DRG 544/545 - inactivate DRG 209 reassign pro codes 81.51,81.52,81.54,81.56,84.26,84.27 and 84.28 to DRG 544 reassign pro codes 81.53 and 81.55 to DRG 545 (note that new pro codes 00.70 thru 00.73 and 00.80 thru 00.84 will also generate DRG 545 if in MDC 8) Modification: added MDC 8 and DRG 544/545 for these codes for FY 2006. (c) New DRG 546 (p.47306) When certain codes are used as primary/secondary diagnoses with a noncervical spinal fusion, DRG 546 will be assigned instead of DRG 497/498. Surgical Hierarchies (p.47312) These were changed for MDC 5 and MDC 8 to account for the new and deleted DRGs. Recalibration of DRG weights (p. 47322) Titles corrected for DRG 14, 276 and 315. V. MISCELLANEOUS MODIFICATIONS. The following Remedy tickets were also resolved by this patch: 1) HD 102030 op/pro codes 41.04 thru 41.09 not giving DRG 481 The solution was to add "B" for bone marrow to the IDENTIFIER (#2)field of the ICD OPERATION/PROCEDURE (#80.1)file. 2) HD 97884 PTF record coded correctly by the facility in Vista Diagnoses 995.90 thru 995.94 are giving DRG 543 instead of 416/417. The solution was to delete references to DRG 543 and change the Major Diagnostic Category back to 18 (INFECTIOUS & PARASITIC) instead of 1 (NERVOUS SYSTEM). 3) HD 106613 Error Testing Patch ICD*18*17 HD 108155 (duplicate) HD 111969 (duplicate) Actually not related to that patch. Pro code 44.67 (and 44.68) when used with a diagnosis from MDC 6 (Digestive System) gives DRG 468 instead of 155. The solution was to modify routine ICDDRG to use the data introduced by Code Set Versioning in ICD*18*14, specifically Multiple 80.1711 instead of Multiple 80.12. 4) HD 107571 New procedure codes missing Major Diagnostic Categories New Procedure codes introduced by patches ICD*18*13 and ICD*18*14 only provided for usage in a single Major Diagnostic Category (MDC). The solution was to add additional MDCs and their associated DRGs as provided in the Federal Register dated August 11, 2004 (p. 49624). 5) HD 112348 Bad DRG Dx 852.06 is giving DRG 28 instead of 27. However, it would be preferable if Medical Records coders were off the system. Install Time - 5 to 10 minutes. Please ensure that your programming variables are set, including your DUZ(0)="@", otherwise installation will abort. This patch does not involve global file restoration. Nonetheless, we suggest you create global saves on files #80 (^ICD9), #80.1 (^ICD0) and #80.2 (^ICD) in the event problems develop during the install. If it became necessary to use the saved files, you should first kill all the records (except the zero node) before applying the global restores. 1. From the Kernel Installation and Distribution System Menu, select the Installation menu. 2. From this menu, you may elect to use the following options: (when prompted for INSTALL NAME, enter ICD*18.0*20) a. Backup a Transport Global - this option will create a backup message of any routines exported with the patch. It will NOT backup any other changes such as DDs or templates. b. Compare Transport Global to Current System - this option will allow you to view all changes that will be made when the patch is installed. It compares all components of the patch (routines, DDs, templates, etc.). c. Verify Checksums in Transport Global - this option will allow you to ensure the integrity of the routines that are in the transport global. Routines ICD1820* can be removed once the patch is installed. d. Print Transport Global - this option will allow you to view the components of the KIDS build. 3. Use the Install Package(s) option and select the package ICD*18.0*20. 4. When prompted, 'Want KIDS to INHIBIT LOGONs during the install? YES//', respond NO 5. When prompted 'Want to DISABLE Scheduled Options, Menu Options, and Protocols? YES//', respond NO. 6. Following installation of this patch, it is necessary to run the [DG PTF UPDATE TRANSFER DRGs] option in order to make the transfer DRGs correspond to the appropriate grouper. This option updates/recalculates the transfer DRGs for PTF records opened during the current fiscal year (since 10/1/2005). The option is locked with the DG PTFSUP security key.
