This patch must be installed at all facilities before updates to the Health Level Seven (HL7) ADT-A24 and ADT-A43 messaging can occur for CHDR on the Master Patient Index (MPI). INSTALLATION REQUIREMENT: From the date of release, MPIF*1*41 has a 7-day installation compliance date. NOTE: MPIF and RG patches should NOT be installed on legacy systems to avoid issues with the legacy systems ending up as Coordinating Master of Records (CMORs) or Treating Facilities. MPIF*1.0*25, MPIF*1.0*35, MPIF*1.0*37, and RG*1.0*42 are required builds for patch MPIF*1*41. Issue #1: Message format changes. In preparation for updating the Master Patient Index (MPI) Health Level Seven (HL7) maintenance messaging, the MPIFA24 and MPIFA43 routines are being modified to address forward and backwards compatible processing of the ADT-A24 and ADT-A43 messages. This is a technical modification to create a consistent and accurate HL7 standard assigning location of identifiers established by the MPI and decentralized systems that communicate with the MPI. Routine MPIFA40 was modified to carry the "from" DFN in the MRG segment. Additionally, a variable was cleaned up and a change was made to request "all" fields instead of selected fields in the PID call. These changes will not affect users of the VistA system. Issue #2: Bogus "AICN" cross-reference. Occasionally, the "AICN" cross-reference remains when it should have been cleaned up. A check has been added to UPDATE^MPIFAPI1 and GETDFN ^MPIF001 to remove a bogus "AICN" cross-reference when it is found to exist and the DFN entry does not exist. This change allows the IMDQ team to use an Austin option - Remote ICN, Checksum and CMOR Update [MPI DATA MGT REMOTE ICN UPDATE] to address a current exception more easily. Test Sites: =========== Bay Pines, FL Central Arkansas HCS Maryland HCS VA Greater Los Angeles HCS Routine Summary: ================ The following is a list of the routine(s) included in this patch. The second line of each of these routine(s) will look like: ;;1.0; MASTER PATIENT INDEX VISTA ;**[Patch List]**;30 Apr 99 CHECK^XTSUMBLD results: Routine Before Patch After Patch Patch List -------- ------------ ------------ ---------- MPIF001 8844271 9040589 1,3,9,16,18,21, 27,33,35,41 MPIFA24 6588472 7211226 22,24,27,31,25,41 MPIFA40 3870753 5346755 22,41 MPIFA43 6125956 10630106 22,41 MPIFAPI1 13147583 13508429 37,41 MPIFP41 N/A 4757433 41 Post-init routine MPIFP41 contains a background job to look for patients with erroneous ICN checksum values in the ICN CHECKSUM (#991.02) field in the PATIENT (#2) file. Any checksums that are inaccurate will be corrected. This job is queued to run in the background. At the test sites, an average of 40,000 patients per minute were processed. Therefore, you can calculate your approximate run time by dividing the number of patients in your PATIENT (#2) file by that average number. When the job is complete, an e-mail message is sent to the installer. At that point, routine MPIFP41 can be deleted from your system. This e-mail message does not include the number of erroneous ICN CHECKSUMS found and corrected, as the number has no significance. An e-mail message is also sent to the Master Patient Index (MPI) with statistics for analysis. FYI - if you run ^XINDEX on routine MPIFA24, a reference to OMGPIDMI is displayed as a routine that is not available on your system and that is correct. This relates only to the Federal Health Information Exchange (FHIE) Framework system. This was introduced in an earlier patch - MPIF*1.0*25. Installation Instructions: ========================== This patch takes less than 3 minutes to install. This patch should be loaded during non-peak hours to minimize disruption to the users. 1. Users are allowed on the system during patch installation. 2. TaskMan does not need to be stopped or placed in a wait state. 3. The HL7 inbound filers should be stopped. Use the Monitor, Start, Stop Filers [HL FILER MONITOR] option on the Filer and Link Management Options [HL MENU FILER LINK MGT] menu off of the HL7 Main Menu [HL MAIN MENU]. 4. These routines are not usually mapped, so you will not have to disable mapping. 5. Use the INSTALL/CHECK MESSAGE option on the PackMan menu. Note: TEXT PRINT/DISPLAY option in the PackMan menu will display the patch text only. 6. From the Kernel Installation and Distribution System Menu, select the Installation menu. 7. From this menu, you may elect to use the following options: (a) Backup a Transport Global. (b) Compare Transport Global to Current System. (c) Verify Checksums in Transport Global. (d) Print Transport Global. 8. Use the Install Package(s) option and select the package MPIF*1.0*41. 9. When prompted 'Want KIDS to INHIBIT LOGONs during the install? ' YES//', respond NO. 10. When prompted 'Want to DISABLE Scheduled Options, Menu Options, and Protocols? YES//', respond Yes and enter the following options: Single Patient Initialization to MPI [MPIF IND MPI LOAD] Display Only Query [MPIF DISPLAY ONLY QUERY TO MPI] Local/Missing ICN Resolution Background Job [MPIF LOC/MIS ICN RES] Coordinating Master of Record (CMOR) Request [MPIF CMOR MGR] 10-10T Registration [DGRPT 10-10T REGISTRATION] Register a Patient [DG REGISTER PATIENT] Load/Edit Patient Data [DG LOAD PATIENT DATA] Electronic 10-10EZ Processing [EAS EZ 1010EZ PROCESSING] 11. If you unmapped routines in step 4, remap them after installation has completed. 12. Restart the HL7 inbound filers. Use the Monitor, Start, Stop Filers [HL FILER MONITOR] option on the Filer and Link Management Options [HL MENU FILER LINK MGT] menu off of the HL7 Main Menu [HL MAIN MENU].
